often thought of as a moving target. It is our intention to ask providers
to inform patients as to their own level of quality of care as well as
survey patients on their treatment experience.
If you have been treated recently, please find your healthcare providers and fill out a MyHealthScore.com on-line survey for your provider and insurance company. When looking for a healtcare provider, users can run a Provider Rating Report to identify the top 25 providers in their state, based upon the surveys entered in MyHealthScore.com during the past 12 months. Please note that there are two survey forms for insurance companies. One designated for patients to fill out, and the other for providers. Surveys are immediately compiled on-line upon submission and may be viewed by provider or insurance companies to evaluate several aspects related to their quality of service. A monthly executive summary of survey information is made available to MyHealthScore.com participating providers on-line to help them better manage their practice.
